We want to make your first visit to us as simple as possible. We accept both self and physician-referred patients. Prior to your first appointment we ask you to gather some necessary information in the privacy of your own home. Please complete each of the printable forms found under our "Patient Forms" page and bring them with you to your first visit.
We are now using 'House Calls' a patient call reminder system. In an effort to provide the highest quality of service, we offer automated courtesy calls to remind you of upcoming appointments.
Additionally, we would like to be notified within 24 hours if you need to cancel an appointment. Please call the office location where you scheduled your appointment. There will be a fee assessed to patients who do not cancel their appointments in advance if unable to keep the scheduled appointment.
Emergencies
A physician is always available to assist you in an emergency 24 hours a day. Please call our office for any urgent needs during regular office hours. After hours our answering service will connect you to an on-call physician and your call will be returned promptly. In a life-threatening situation please go to your nearest hospital emergency department. After any emergency visit, please notify our office so we can provide the necessary follow-up.
Referrals
If you are a member of a Health Maintenance Organization (HMO), you may be required by the plan to get a referral from your primary care physician before visiting a specialist. Please contact your primary care physician in advance of your appointment to obtain the necessary paperwork. They may need several days to process your request.
Insurance Information
Please bring your insurance cards with you to each visit so we can copy them for our records or update our records as needed. We participate with most major insurance plans. To verify that our office participates with your plan contact the toll-free number on the back of your card.
